The Shadow Temple in Ocarina of Time. Might not be as bad to play now that I’m an adult. But kid me hated going anywhere near it. The moans of the Redeads were enough to cause nightmares.
As an aside – it’s a great piece of dark history fleshed out in an otherwise tonally different game.

Vampire: The Masquerade – Bloodlines. It’s an older game but the ocean house hotel level freaked me the fuck out the first time I played through it. Makes you forget that you’re actually playing a vampire.

The dead lands in Bug Fables. There is a giant creature watching out for you in the background. If it spots you it will try to drop a smaller but still difficult enemy on top of you, like it’s toying with you and wants to watch you fight for its amusement. You’ll never fight the giant though or get a good look at it, you’ll only see its eyes and arms.

In one of the older Mario games( I forgot which one though) there was a secret level that brought you to this arena that was filled with boos and with I think an old broken ship. Its a pretty creepy secret in a old non-horror game such as Mario.
