Herdy Gerdy was a game which I felt this. I never played this as a kid, would always see it at game shops but always freaked me out given the box art seemed like a horror like game where you were chased by a monster or people (I assumed you played as the little creature at the bottom)
Played it recently as I picked up a ps2 bundle and the game was included. Game is completely different to what you see on the front cover. Definitely felt somewhat surprised. But got me thinking, what games have you seen a front cover and thought to yourself “This game must be about that” but once you play it for yourself you see it’s a completely different game that what the cover vibes gives off.

The North American cover of Ico gave me the vibe that it would suck, but it turned out to be one of my favorite games of all time. But I did print out the correct cover art to cover up that monstrosity.
Super Robot Wars Impact, and by extension, the entire Super Robot Wars series. I thought the game with a bunch of giant robots on it would be an action game of some sort, but in reality, it’s actually a tactical RPG. I had no clue how to play it when I was a kid, but I figured out how to play when I was a preteen and it has since become one of my favorite game franchises.
Oh man, memory unlocked. That game’s box art specifically always gave me the creeps! I never even dared to look at it, let alone considered buying it, just for that alone.
played this as a kid, brother got it for Bday gift, sold it to a gamestop years later, when like within the last 3 years i bought it off of ebay because i wanted it.
kooky game, neat music but fps is shit in some areas, also soem wonky choices for some VA’s like the elf guy. WOW bad choice.
