My guess is they just use default npc body and added layers on top of it. In lore I guess they screw cyberware straight through skull

Does it mean that, from the developer perspective, Maelstrom models are essentially a scaled down human model, wearing a costume of scaled up human model, with face implantation just being helmets? If that’s true, it even beats metro heads from Fallout 3!
